Deep gluteal syndrome, formerly referred to as piriformis syndrome, is an overuse syndrome characterized by buttock pain accompanied by symptoms of sciatica. The condition is often caused by the sciatic nerve becoming entrapped and impinged around the piriformis muscle in the sub-gluteal space.

Because treatment for sciatica originating in the lumbar spine is sometimes invasive, it is important to rule out deep gluteal syndrome as the source of pain. The Piriformis Test will reveal whether the pain is originating in the sub-gluteal space at the piriformis muscle versus the lumbar spine.
Deep gluteal syndrome can usually be treated with physical therapy, chiropractic care and anti-inflammatory medications. Consistent stretching of the piriformis to relieve tightness will alleviate pain over time.
